Maggots is one of the 35 possible corruptions that can be inflicted on the PC. While the corruption is in effect, at random intervals a tame worm will spawn from the PC's body; this causes a few points of damage, thus in very rare situations can actually be fatal.

Though companions can be put to tactical uses by some players, the Toughness loss afflicted on the PC by the corruption is a very significant problem for many of them. Chaos Knights starting with the corruption may face a slightly more difficult early game; on the other hand, the worm companions may possibly be used to at least create a distraction in desperate situations, and Chaos Knights start with plenty of PV due to their armor anyway.

Apparently, spawned worms have their level adjusted based on the number of worms of the same color that the PC killed thus far (as per the KPL formula) — meaning that late in the game (or otherwise after long worm killing sprees), the worm companions sprouted can be very strong allies indeed.
